  • facilitate conversations with your family and friends, so they are aware of your wishes and can participate in your end-of-life care

The Process

First visit — 90+ minutes

Through an in-depth conversation about your unique situation, values, and beliefs, we will identify the medical and non-medical care you would like to receive before, during, and after your death. This can include planning a home funeral, if you wish. I will suggest ‘next steps’ and refer you to people who can complete any necessary paperwork that we can’t do together. If more than one session is needed, we will arrange it at no additional cost.

After our visit

I will compile your holistic end-of-life plan into an easy-to-read document and email it to you for review. We can chat about any changes or clarifications you would like to make.

If you would like, we will arrange a family facilitation meeting to introduce your plan to friends and family (hourly rate applies).

two people’s plans – $750 + gst
